Please submit this form as soon as possible, so that we can plan for the coming school year.

• This form will ask you to confirm either that your scholar is returning in 2025-26 or that you are withdrawing them at the end of this school year. 
• Each scholar currently enrolled at Prodeo is assured a spot for next year.
• If you have multiple scholars at Prodeo, please submit this form for each scholar.
