Waiver of Immunization Requirement: Parents seeking to exempt their children from Minnesota's vaccination requirements are advised to connect with your school's main office. The school's health office will provide guidance on obtaining, completing, and notarizing the Immunization Exemption form.
If your scholar has any health needs, please complete the appropriate forms; these forms can be faxed from your clinic, emailed (with necessary signatures), or brought to the school office.
